Mom's Meals®, USAging Partner to Honor Outstanding Case Managers With Community Champion Award

Mom’s Meals and USAging today announced their partnership in the Community Champion Award, which applauds Area Agency on Aging and Title VI Native American Aging Program case managers for raising awareness about nutrition in their local communities and among the clients they serve.

Mom’s Meals, a leading provider of medically tailored, home-delivered meals, and USAging, the national association representing and supporting the national network of Area Agencies on Aging, also announced that the nominations period is now open for the 2022 Community Champion Award.

The Community Champion Award, first introduced by Mom’s Meals in 2020, honors case managers for their outstanding achievements in the areas of nutrition, health, and healthy lifestyles. The award underscores the value of the case manager’s role in supporting whole health in their communities and promotes awareness of the importance of nutrition in overall health—particularly to reduce the impact of social determinants, to support individuals who want to age at home, and to support the management of chronic conditions.

Nominations are currently being accepted for this year’s award, with a winner to be announced by Mom’s Meals and USAging at the USAging 47th Annual Conference and Tradeshow, taking place July 10-13 in Austin, TX. Case managers are nominated by their peers or supervisors. Nominees d>o not have to be contracted with Mom’s Meals to be eligible; however, they must be from a USAging member agency. The last day for nominations is Tuesday, May 3.
